Transition from children's services

Transition can be any period of change in a person's life.

For a young person with a disability or a long-term health condition it is the time when they move towards adulthood. Each individual may be ready for this transition at different ages. However from a legal perspective a young person is considered an adult at 18 years of age.

To ensure that this process is successful, transition planning is important and can start as early as 14 years of age. Adult Social Care works closely with colleagues in Children’s Services to facilitate this.
