NHS South West London Health and Care Partnership has developed a Long-Term Conditions (Cardiovascular, Respiratory and Diabetes) toolkit, including resources in other languages to improve accessibility for individuals from different communities for whom English is not their first language. This also ensures individuals are able to look after their health and wellbeing during the pandemic and longer-term.
