Report a quality of care concern

Tell us about an issue with an adult social care provider’s service which is not abuse or neglect.

We’re working together with people who use services and the service providers who deliver care to make adult social care the best it can be. We use these reports to monitor care providers to make sure they’re providing safe, effective, and high-quality care.

What is considered a quality of care issue

You can tell us about anything you think is affecting the quality of care someone is getting from a carer or care home. This includes:

  • not treating people with respect or dignity
  • missed or late calls
  • any other concerns about the quality of care people are receiving

If you’re worried that someone is being hurt, abused or neglected, or at risk of this happening, report it as a safeguarding concern.

What you need to tell us

We’ll ask for:

  • name of the care provider
  • what type of care they provide
  • details about the concerns you’ve noticed
  • details of the person receiving care, if relevant
  • your name and contact details

Staying anonymous

We need your name and contact details to talk to you about your concern, but we don't have to pass them on to the care provider or staff member that you're telling us about. We ask in the form whether you’re comfortable with us sharing your name with them.

It may be harder to investigate your concern if we can't share your name with the care provider or staff member.

What happens after you make a report

  • Your report will be sent to our Quality Assurance team.
  • If you've given us your email address, we'll send you a confirmation email.
  • We'll contact you if we need more information.
  • We won't give your name to the care provider or support staff unless you've given us permission.
  • We aim to get back to you with an update from the care provider within a month.
