What's the point of this survey?
In the face of recent Government cuts, managers in the NHS and councils across South West London are looking at the health information available to under-18's. To help us do that we want to know what you think about the information and advice that's available so we can plan services to meet your needs.
Please take the time to fill in this short 11 question survey. In return we will help you out with your Christmas shopping by entering you into a prize draw to win one of three £25 Amazon gift vouchers. You must answer all questions to be entered.
