Part 1 - Your details
|
This is the easy part – your name and present address, as well as that of your partner if you have one. Please remember to put the date you moved in as this may affect whether you are eligible or your priority with some of the Kingston Housing Partnership.

Part 2 - Details of your household
|
List everyone who is part of your household and who you want to be rehoused. Citizenship and immigration status To help us decide your eligibility for rehousing, please provide copies of the following documents for each member of your household: - All pages from your passport.
- Letters from the Immigration and Nationality
Department concerning your immigration status.
- Any other documents which establish your
immigration status.

Part 3 - Past Addresses
|
Please provide as much detail as you can about your past addresses.
|

|
|
Part 4 - Present address
|
How you fill in this part of the form will help to decide your priority for a move. If you are separated from a member of your household, please base your answers on the address which provides the most rooms or facilities for your household.

|
|
Part 5 - Pets
|
Many of the properties owned by members of the Kingston Housing Partnership have policies relating to pet ownership. We do not allow cats or dogs in any properties which do not have sole use of, and direct access, to a garden. Refusing a suitable property because you have a cat or a dog will not be accepted as reasonable grounds for refusal.

Part 6 - Your links with the Royal Borough of Kingston upon Thames
|
Please fill in this section as fully as possible. If you are applying as a homeless household this information may determine whether you have a local connection with the borough.
|

|
|
|
Part 7 - Income and savings
|
If you are applying as a homeless household this information may be needed to determine whether the borough has a legal duty to provide you with temporary accommodation. This information may be needed by members of the Kingston Housing Partnership as some operate income restrictions on who they can house. If you have indicated that you are interested in home ownership elsewhere on the form, this information is also used to assess whether your income is sufficient to meet the costs associated with home ownership.

Part 8 - Your housing choices
|
Please indicate the types of properties and areas you want to be considered for. Due to the shortage of certain types of properties we cannot guarantee to rehouse you in your chosen type of property or area and if you are very limited in your choice of properties or areas, it may be very difficult to rehouse you.

|
|
Part 9 - Medical or social conditions affected by your housing
|
You may wish to tell us about any medical condition experienced by you or any member of your household that is affected by your housing. Assessments will only be carried out for anyone included as part of your housing application. Because someone is seriously ill it does not necessarily mean that medical priority will be awarded. It will depend on whether the medical problem is made worse by your housing circumstances, and the degree to which this is happening. Part 10 - Homeswaps
|
This section is relevant to council and housing association tenants only. Please note that Homeswaps was suspended by the Government in January 2007.

Part 11 - Home ownership
|
From time to time low cost home ownership schemes are developed which operate on a part-rent part-buy basis. These are known as shared ownership and are normally built by housing associations. You buy a share of the property, usually in the region of 25% and pay rent to the housing association for the remaining share. You need to be able to finance a mortgage as well as legal and other fees associated with buying a home, including paying rent for the share that is owned by the housing association, and carrying out repairs. If you have indicated that you are interested in this type of scheme, depending on your level of priority on the Kingston Housing Register, you may be contacted by housing associations operating these types of schemes. Other home ownership schemes are sometimes available where it is possible to obtain a grant or low-cost loan to assist with buying. Applicants on the Register are notified as and when these opportunities arise.

Part 12 - Relatives employed by the Kingston Housing Partnership
|
This question is to ensure that properties are not being allocated unfairly because of family links with employees of the Kingston Housing Partnership.

|
Part 13 - Equality monitoring
|
Please fill in the equality monitoring insert in the middle of the application form. The questions on the form are for monitoring purposes only to help us provide better ser vices and will not affect your application for housing.

Part 14 - Extra information
|
This section is for you to tell us about anything that you feel is relevant to your housing situation that you have not already told us about elsewhere on your form. For example if you have been asked to leave your present home or have been issued with a Notice to Quit or Possession Order.

|
|
|
Part 15 - Declaration
|
This section is to alert you to the fact that it is a criminal offence to give false information to gain a tenancy. Please ensure that you have filled in all sections of the form and signed it before returning it to us. We cannot accept unsigned application forms.
