The expectation is that children and young people with disabilities in Kingston will have the same access and opportunities to services, to help them to be healthier, safer and to enjoy and achieve, ensuring the future economic well being of disabled young people and that they have a voice in shaping the right services to meet their needs.
The Moor Lane Children and Young People's Development Service will provide co-ordinated treatment, help and support in order to:
- Close achievement gaps and tackle inequalities by focusing prevention and early intervention strategies more effectively
- Reduce vulnerability and risks for children and families by strengthening front-line holistic services in Extended Schools, Children’s Centres and universal provision
- Improve health and well-being for children with disabilities, and other vulnerable groups, by integrating our services and pooling resources utilising multi-agency, co-ordination and participation where necessary.
The aims of the service are to:
- To provide timely multidisciplinary assessment of children and young people with disabilities, using a single assessment process
- To provide clear information for children, young people and their families on the range of services available that includes threshold criteria and access arrangements
- To record and present activity and quality information as required by the Commissioners and other statutory agencies
- Children and young people who are disabled or who have complex health needs, will be supported to participate in family and community activities and facilities
- Health, and Prevention and Safeguarding Services will be organised around the needs of children and young people and their families. There will be co-ordinated multi agency assessments leading to prompt, convenient, responsive interventions that maximise the child’s ability to reach his or her full potential
- Children and young people and their families will be actively involved in all decisions affecting them and in shaping local services
- Through direct provision or commissioning, provide services that reduce the need for external placements and reduce unscheduled admission to hospital
- To provide services in accordance with locally and national key performance indicators, recognised best practise and other targets
- To provide a range of short break provision that promotes inclusion
